Scholarship Form

Forms must be completed in full and a form must be completed for each scholarship recipient. Full, partial and payment plans are available for scholarships. All fees not covered by financial aid are the responsibility of the recipient. Forms must be submitted at the time of registration. All financial aid granted must be paid back before a release to another club is granted.

Three times and field locations must be submitted each season. Practice locations are not guarantees each season and seniority is given based on level, age and submission date. Please do not assume you have the same practice time each season.
